Who Qualifies to apply for Birth Services:
The person who is the subject of the certificate; A parent of the person who is subject to the certificate, if that person is less than 19 years of age

If you are acting on behalf of an individual who has provided you with authorization to order their document under section 36(1)(d) of the Vital Statistics Act, you must submit your application along with the original letter of authorization to the Vital Statistics Agency by mail. You may not utilize this electronic ordering system for that purpose. If you are applying for a Certificate or Certified Copy of a Registration for yourself, select 'SELF'. If you are applying for a Certificate or Certified Copy of a Registration for your child, indicate whether you are the Mother or the Father.
